Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Regulations.

You will have digital access to your medical records if you have signed up for digital services at Patient Access and/or MyHealthcare App. This will give you access to you medical records as and when you require them and will be a live account of the records we hold for you meaning they are up to date. Please be aware that if you are a new patient to our Practice it may take time for your medical records to reach us from your previous Practice.

If you only require a one off copy of something from your records and not a constant up to date version you can complete this request form: Patient Access to Medical Records Request Form.

Please be aware all requests can take up to one month to process.

NHS Care Records

The NHS in England is introducing the NHS Care Records Service (NHS CRS). Over time, the NHS CRS will begin to provide healthcare staff with quicker access to reliable information about you to help with your treatment, including in an emergency.
GP practices in South Birmingham are amongst the first in England to begin implementing the new NHS Care Records Service as part of an ‘Early Adopter programme’. ‘Summary Care Records’ are being created for South Birmingham patients.Existing patients of this practice will already have received information about the service.  Newly registering patients will be given information when they attend the practice to register.  Patients can limit their participation and also opt out of having a Summary Care Record. If you choose to opt out, you need to visit surgery to speak with a receptionist and sign the necessary form.
