Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Regulations.
You will have digital access to your medical records if you have signed up for digital services at Patient Access and/or MyHealthcare App. This will give you access to you medical records as and when you require them and will be a live account of the records we hold for you meaning they are up to date. Please be aware that if you are a new patient to our Practice it may take time for your medical records to reach us from your previous Practice.
If you only require a one off copy of something from your records and not a constant up to date version you can complete this request form: Patient Access to Medical Records Request Form.
Please be aware all requests can take up to one month to process.
