If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £30.00.
Many employers have their own self-certification forms. If your employer doesn’t have its own form you can download the Self Certification Form. Please print it, fill it in and hand it in to your employer. You do not need to see a Doctor.
If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness To Work’ Certificates. You will need to have seen a Doctor for the illness to be issued with a certificate. If this is an ongoing illness you can request further certificates via a telephone consultation however the Doctor may wish to see you to review your symptoms before issuing a certificate.
Referrals
Most referrals to other specialists will require you seeing or speaking with a GP. There are some local services which you can refer directly into.

If you have already been referred by your GP and you need to track your referral you can contact the hospital directly. Most of our referrals are completed via the NHS e-Referral Service (Choose and Book). You will have been given/sent the paperwork regarding your referral, you can use this information to contact the hospital.
Test Results
If you had tests at the Practice and you have signed up to online access you can view your results by logging on to your chosen service here: MyHealthcare App or Patient Access
If you would like to speak to some to get your results our reception team can read out comments left by GPs about your results Monday to Friday 9.00am-6.15pm. If the GP has not yet commented on the results our Nurses are available to give out results between 1.00-1.30pm Tuesday, Wednesday, Thursday & Fridays. If you require more information about your results you can request a telephone consultation with a GP, our reception team can book this for you.
If you had tests elsewhere you can chase your results directly with the hospital or clinic.
