If you are a carer or have a carer you should inform your GP Practice of this.
For our patients and their carers please complete the Carers Form opposite.

To access carer support services please contact the Birmingham Carers Hub. They will advise you on relevant services that you require; including carers needs assessment service, training, support groups, information and advice, emergency back up support in the event of you being unable to provide care and lots more.
